IMPORTANT INFORMATION

The surgery is open Monday to Friday from 8am to 6:30pm. If you have an urgent problem during these times please ring the surgery. At all other times ring NHS 111.

Requests via eConsult for non-urgent problems can be made between 8am – 6:30pm, Monday to Friday. The practice will respond within 2 working days.

Due to COVID we are operating what is known as total triage, this is a virtual appointment. Your GP or nurse will review your problem and will then, depending on your needs

  • offer you advice via text
  • offer you a telephone call
  • offer you a video call
  • ask to see you in person (face to face) or
  • navigate you to other services

We no longer take prescriptions over the telephone – please use your online access or email the surgery warccg.brookfieldpm@nhs.net or post your request to the practice.

When attending the surgery- It’s important that you arrive at the time we ask you to attend and don’t forget to wear a face covering or mask.

In line with social distancing rules the surgery must limit the number of patients in the waiting room.


The first phase of the NHS Covid-19 vaccination programme has started.

When it is your time to receive your vaccine you will receive an invitation to come forward. Please do not contact the NHS for an appointment until you are contacted.

The NHS is prioritising the rollout of the COVID-19 vaccine. When it is your turn to get the vaccine you will be contacted. This may be by phone or letter from your GP or national NHS booking service.

Want advice from your doctor, self-help information or have an administrative request such as a sick note or test results? eConsult and get a response before the end of the next working day or sooner

eConsults will be responded to by the end of the next working day. If you have an urgent issue please ring the practice (in hours) or NHS 111 (out of hours). You can submit an econsult between 8.00am – 6.30pm Monday to Friday only. Following clinical triage we may allocate you an telephone appointment slot or face to face appointment up to 4 weeks in advance of your E-consultation.

The Practice does not accept prescription requests over the telephone. Please order using your online access, email warccg.brookfieldpm@nhs.net or post your request.

Welcome To The Practice

Brookfield Surgery continues to provide services as we have been doing throughout the Covid-19 pandemic and are prioritising delivering care and services based on clinical need.  Currently the Surgery is dealing with unprecedented demand whilst also trying to support NHS England in delivering the vaccination process for COVID-19

If you need medical advice or treatment please consider whether you enquiry maybe answered via the NHS website, 111 or at the local pharmacy. All enquiries should continue to be submitted via the Econsultation link available on this page. The information submitted helps the surgery prioritise the clinical urgency and allows you to give the Clinician as much information. Incomplete forms only with minimal information only delay response times. Telephone enquires limit the response of both the receptionists and clinicians and can lead to long wait times which is frustrating for everyone.

Due to coronavirus how you contact us will be different at the moment.  This is to help stop the spread of coronavirus to other patients and the team. When you phone or go online you will be asked to provide some information about your healthcare need.  This will help the practice healthcare team to assess the most appropriate way to provide your care that may include:

· a response via econsult giving you information and directing to a more appropriate resource

·a telephone/video consultation

· a face to face consultation with a healthcare professional

Face to face appointments are available to all patients where there is a clinical need and these will be assessed by the GP when they contact you.  If needed, you will be asked to attend the surgery in person to see a health care professional that day.

Online consultation requests for non-urgent help can be made Monday to Friday 8am – 6.30pm only.  They will be responded to as soon as possible, usually within two working days.  Alternatively visit the local pharmacy for clinical advice on minor health concerns.

For urgent issues or out of hours you can also call the NHS 111 or go online to seek NHS advice 111.nhs.uk.

 

Covid-19 Notice

We ask that all patients attending the surgery must wear a face mask or face covering.

This does not need to be of medical standard. A cloth face covering should cover your mouth and nose while allowing you to breathe comfortably. It can be as simple as a scarf or bandana that ties behind the head.

You should wash your hands or use hand sanitiser before putting your face covering on and after taking it off – It is important that you don’t touch your face covering when wearing it, where possible, to avoid hand to mask transmission of the virus.

PLEASE ENSURE YOU HAVE A NOMINATED CHEMIST. RING THE SURGERY IF YOU  NEED TO ARRANGE A NOMINATED CHEMIST. IF YOU DO NOT HAVE INTERNET ACCESS ASK A FRIEND OR FAMILY MEMBER TO REQUEST THE PRESCRIPTION USING THE EMAIL ADDRESS.  PLEASE RING THE SURGERY IF YOU HAVE ANY DIFFICULTIES


Due to ongoing concerns around COVID-19 (Coronavirus) we are temporarily suspending the facility for patients to book online appointments at the surgery. Please submit an eConsult (https://brookfieldsurgery.webgp.com/).  This is to ensure that we are able to fully assess risk before appointments are booked at the surgery.

The practice is taking measures to reduce risk to our Patients and Staff members with regards to rising concerns about COVID-19.  Having lots of people in any GP waiting room is not going to be very healthy, especially for our more vulnerable patients.  We are asking patients to not come to the surgery to book appointments or for queries.  Please submit an eConsult via the link below or telephone our receptionists for a query.

We are also reducing the number of people attending for face to face appointments, using telephone or video consultation as alternatives.

  • Thursday 30th September 1pm – 6.30pm
  • Wednesday 20th October 1pm – 6.30pm
  • Thursday 25th November 1pm – 6.30pm

Please note that this practice will be closed from 1pm for our Staff training Protected Learning Time (PLT).

Emergency cover will be provided by PC24 who provide the Out Of Hours service, if you have a medical need during this time please dial 111 or if it is an emergency then please call 999.

The PLT days have been suspended over the last 12 mths or so due to the Coronavirus pandemic but they have been reinstated for the next 12mths. 

They may change day and week from month to month and this year are not always on the last Thursday of the month. We will post the days when we will be closed on this page.
